I’d love to say it was from doing backflips in front of a cheering crowd.
I’d love to say I went out in a blaze of glory.
But the reality was…much more embarrassing.
I was actually just going off the lip of a half pipe. As my momentum carried me into the air, I twisted my body to initiate the spin. My arms were outstretched.
As I twisted, I felt and heard a clunk in my right shoulder. The humerus (the upper arm bone) shifted, and I gagged from the pain. I immediately lost control and then landed, painfully, face first into the snow. The pain in my shoulder was excruciating. My shoulder, back, and arm all burned in a way I had never experienced before…and never hope to repeat.
That was the beginning of years of shoulder problems.

The worst part was…nothing seemed to work.
- I tried massage.
- I tried physical therapy.

I spent countless painful hours struggling with my shoulder. Multiple trips to the doctor, intensive physical therapy sessions, and countless anti-inflammatories. In the end, it felt like I was wasting my time, money, and health on ineffective exercises that gave me no results.
Weeks turned into months. I just couldn't do any of the things that I wanted to do. Doctors told me to just rest. I rested and rested …and rested. Within one year, my hands, wrists, and elbows were hurting too!
Months turned into years. I visited massage therapists, chiropractors, acupuncturists, and physical therapists. From time to time, I’d get a few hours of relief. My shoulder would feel a little better for a while, but as soon as I went back to the computer or tried to play hockey, the shoulder would feel sloppy and unstable. It felt like the ball just wasn’t in the right place in the socket.
Even if the acupuncturist or massage therapist could get it to feel right, within hours I’d be back at square one. The aching would return. The popping would be there. The sense that my arm was just dangling kept coming back.
Sleeping on my side was out of the question, as the shoulder would give me a sharp pain deep in the socket along with a nauseating feeling of something being loose and clunky in the joint.
It’s unfortunately quite common for people to feel like what they have done in physical therapy, chiropractic, or massage wasn’t particularly helpful. A lot of times, conventional approaches to shoulder problems are based on avoidance. Everyone's REALLY afraid to hurt you more!
Of course you don't want to get hurt more...so they tell you to NOT MOVE until pain goes away. But if you're like me, you've found “rest, rest, rest” to be a really frustrating and fruitless approach.
A note about rest and shoulder pain...
It's a common misconception that rest will heal pain. Sometimes rest does help, but when months and years of rest don't lead to better function and less pain, it's time to start moving!
Historically, people with back pain were told to just rest in bed and wait for the back pain to go away. It took decades for the recommendations to change, but now doctors suggest moving to make sure your back muscles don't get stiff and weak!
It's no different with shoulders. The only way to improve your ability to move is to move!
But of course, not all types of movement will help. Conventional approaches to shoulder issues are often focused on doing the wrong things to the wrong parts of the shoulder — or they target the right parts of the shoulder but in an ineffective way! These approaches cause more harm than good!
EXAMPLE #1: GETTING THE WRONG MUSCLES TO RELAX!
If you take a look at many shoulder rehab protocols from reputable sources, you’ll find what I found: a lot of the exercises don’t feel good in the short term and don’t seem to do much in the long term.
This Pendulum exercise, for example, is one that seems to appear in nearly all physical therapy protocols for shoulder pain. I remember being told to do this exercise to help my shoulder.
It didn’t feel good then, and it didn’t do any good. It's not that I was a bad patient. It's not that I didn't do it.
The problem was, I was given a one-size-fits-all approach that worked out the wrong things! This exercise was making things less stable for me. Each time I did it, my shoulder felt WORSE.
EXAMPLE #2: IGNORING POSTURE — OR WORKING IT THE WRONG WAY!
Many programs also completely neglect the importance of the posture and alignment of the entire body — something that practically guarantees more shoulder pain in the long term.
Some programs do pay attention to posture, but then they often approach it with ineffective exercises.
Take this exercise for example. The idea behind an exercise like this is to “fix” your posture, but this doesn’t actually fix your posture! It completely misses the right muscles and can actually put your spine into a worse position and make your shoulders feel worse!
You aren’t alone. I’ve definitely felt that way. I've worked with a lot of clients who were CONVINCED that surgery was their LAST HOPE.
When everyone tells you that your shoulder pain will go away on its own and it doesn’t.
When you try everything to avoid surgery and nothing works.
...And then, when everyone tells you that your continued shoulder pain means it’s serious… and you need surgery…
It's really easy to start thinking that it’s the only path you have left.
I'm not a doctor.
I can't tell you with 100% certainty that you don't need surgery (some people MIGHT)...But I can share some stories and ideas that you may find very encouraging.

X-rays and MRIs can be really, really scary. When a doctor tells you that what you see in an X-ray is causing your problems, it’s natural to assume they know what they’re talking about. They ARE a doctor, after all.
But here’s the thing.
When you look into clinical studies on X-rays and MRIs for shoulder issues, you start to see something crazy. Just because an MRI or X-Ray shows a person has labral tears, odd bone shapes, rotator cuff tears, or arthritis…
Doesn’t mean they experience the symptoms or PAIN.
MRIs and X-Rays are amazing tools…but they have their limits!
And when you look at real cases of shoulder pain, you find that the MRIs don't tell you the full picture (check out the video for a striking example!)
Over the years, it has become common belief that rotator cuff tears, labral tears, and misshapen bones in the shoulder are the primary causes of shoulder pain...but a relentless army of recent, peer-reviewed medical research has shown this just isn’t true!
Early studies on orthopedic surgeries always paint a rosy picture. Once the surgeries are compared to other treatments, the results are not as great.
Surgery is usually presented as a practical solution to the problem. “Go into the body and directly fix what’s wrong.”
There’s just one problem.
Surgery is unpredictable. It’s simple on paper, but things are rarely so straightforward in real life. Some people get better, and others end up going back for one operation after another until their bodies just can’t take any more.
A study published by the National Institute of Health showed that out of a group of 154 patients who underwent total shoulder replacement surgery, 19.5% had to change their occupations because of the side effects of the surgery.
Studies have shown time and time again that surgery can be a unreliable solution, and that exercise can be just as effective.
Many times I considered going to a doctor to get surgery. But two things stopped me from doing that.
First, the American health care system was extremely expensive at the time (and still is at the time of writing), and I didn’t have insurance that would do much to help cover the cost of surgery.
The second problem was that I had trouble believing the surgery was going to fix me.
I knew a guy from my hockey team who had a bum shoulder. He had slammed into the boards multiple times over his career, and his shoulder was pretty unstable as a result. He was told a rotator cuff repair was his best option, so he went for it.
By the time I was thinking about surgery, he’d already undergone four revisions on that shoulder, and his shoulder still wasn't stable, secure, and healthy.
Another friend from high school also had some shoulder issues. He went in for shoulder surgery. When the smoke cleared, he couldn’t externally rotate at the shoulder joint. While his shoulder was more stable in general, he’d lost the ability to play sports.
Surgery didn't seem to be a good bet, so I kept searching for answers.
These diagnoses can get you feeling anxious, trapped, and resigned.

Focus on the muscles - not the diagnosis!
Medical approaches often focus on "what's broken?" Medical investigation and treatment focuses on finding all the broken things and numbing them or "fixing" them. But, like we've seen, the X-rays and MRIs don't actually mean much! So a lot of times the wrong stuff gets "fixed"!

I was ready to have surgery on my shoulder after four years of pain and not getting anywhere with the various methods I'd tried. My pain started out as a feeling of tearing in my upper right trapezius. It had come on gradually even though I hadn't had any accidents. I was ignoring the pain, and it wasn't going away.
Then, I slipped on some ice on my back step. I caught myself on my right hand as I fell, jamming my shoulder. That put me in so much pain I was forced to see a doctor. The doctor sent me to physical therapy, which helped me at least gain mobility and lessened the pain over time. I’m an artist, and I work on large canvases. Working with large pieces of art was painful. I spent a lot of time and money trying to learn to paint differently, and I wasn’t able to find an alternative. So, my work suffered along with my body.
After the initial physical therapy visits, I was getting better, but the pain was still always there. Over time it got worse. Several other physical therapists helped, but again, I was not able to restore the strength and mobility enough to get on with life.
I went to an orthopedic surgeon. He took an x-ray and told me they could go in and scrape off some of the bone to lessen the pain, but I could manage with P.T. until I was ready to have surgery. I decided to wait and just deal with the pain.
After another year or so, I couldn’t handle it anymore and thought I was ready to have surgery. When I was doing some reading online about what an MRI entails, I came across Matt’s Shoulder Fix program. That seemed to be an answer to my prayer. Since it was not a very expensive program, I thought I’d give it a try. I have so much more hope now than ever before. I still can have days when I overdo it, but then I just go back to more tissue work and take it easy with the weights. This method is working quicker than anything. My advice to you is to try this! It’s worth the small amount of money!

First and foremost, you should make sure you’ve had ample time to recover from your surgery and that your doctor has cleared you to exercise your shoulder through many different ranges of motion and to massage the muscles around the shoulder. If you have the okay from your medical team, you should be good to go.
It’s important that you be patient and careful as you go through the program. Surgery can affect your joint motion in ways we can’t possibly know or predict, so you’ll just need to be mindful as you try different exercises. Just make sure you never push through sharp pain, and if an exercise makes things obviously worse, move on to a different exercise or approach!
Just because you have a scary sounding diagnosis does not mean you cannot improve things drastically!
While all these medical diagnoses are often blamed as the cause for pain, these issues are commonly found in people with no pain. A lot of recent research seems to indicate that these diagnoses have little to no relationship with pain or movement problems.
For that reason (and many others), we encourage people to focus on dealing with the muscles and with restoring good movement patterns to reduce pain and discomfort. If you spend a lot of time working on that and you still run into roadblocks, then think about more drastic measures.

I identified posture as an opportunity for a lifestyle change. I spend a good amount of time at a desk and definitely notice that my shoulders slouch. I have been actively engaged in improving my posture and performing periodic scapular retractions throughout the workday. "So, briefly about myself, I am a 60-year-old physician, and if I can’t fix myself with non acute musculoskeletal issues, I don’t usually go to the doctor.
